5th Year Permit Approval

All approved PBVS institutions must submit a renewal application with the appropriate fee each year following the issuance of the initial permit of approval other than the year the Fifth Year Permit of Approval application is required. The Certificate of Approval expiration date determines when the Annual Renewal or Permit of Approval application must be submitted to IBHE. Please see the information below.

If your certificate expires on June 30 or August 30 your application is due April 25.
If your certificate expires on October 30 or December 30 your application is due June 25.

Application Instructions

The following links will open up the documents you need to complete and submit to IBHE to apply for the annual renewal. You can save the documents in one of two ways: either (1) right click and select “save link as” or (2) click on the link opening the document and then save the blank application files to your computer.

Once you complete all items, follow the instructions in the document “Application” under the “General Information” section that explains how to submit them as a package to the IBHE for consideration.

Note that fees are assessed for the review of applications. The fees were approved so that IBHE would have the staff to review applications and complete PBVS oversight activities. Payment of the fee does not guarantee approval of the annual renewal.
